How to do it

  1. 1On the edge of a step, rise up onto both toes, shift all your weight to the injured leg, then lower that heel below the step over a slow three-second count.
  2. 2Step back up with both legs and repeat.
  3. 3This heavy-slow eccentric loading is the best-evidenced treatment for Achilles tendinopathy — mild discomfort during the exercise is expected and acceptable, sharp pain is not.
  4. 4Consistency over 8–12 weeks is what makes it work.
